Your First Visit
Getting started at our clinic is easy and our staff will be there to guide you through every step of the process. The three most common types of visits are:

The first step is to fill out one of our intake forms. We ask that you arrive 5-10 minutes prior to your scheduled appointment to complete this form, or if you prefer you may do so at home.

If you are going through your health insurance, please bring your card and applicable information so that one of our staff may verify your benefits and chiropractic coverage.
If you have suffered an injury from an accident, we will help you through the entire claim process. Most folks do not know that you have up to one year to file a claim and PIP (Personal Injury Protection Policy) provides up to $15,000 in medical coverage. To clarify, medical care is covered by auto insurance, not group health benefits, at no out of pocket expense to you. PIP coverage is a staple of every basic policy in the state of Oregon. If neither verifying benefits nor filing a claim applies to your needs, we also offer time of service rates for our uninsured patients. The next step is to call our office (503) 719-7742 to schedule an appointment time.
Please plan an approximate time of 60 minutes for your first visit. This will cover a discussion with Dr. Raeburn of your current health concerns, an overview of your personal history, an initial exam which will assess your condition, followed by your first treatment.

At this point, it may be indicated that imaging is required. If this is the case, arrangements will be made for you at a preferred facility. At your next follow-up visit, we will review your exam findings in detail and discuss treatment options, cost, and duration of care.
